These terms apply to consultations and treatments booked through Rejuvence Aesthetics. Please read them before your appointment, especially the sections on deposits, cancellations and your right to cancel. Nothing in these terms affects your legal rights as a consumer.
Rejuvence Aesthetics is a trading name of Facial Aesthetic & Cosmetic Enhancement Clinic Limited (trading as Rejuvence Clinic and Rejuvence Aesthetics), a company registered in England and Wales, company number 08717446. Our registered office is 36 Lichfield Street, Walsall WS1 1TJ.
Our clinic is at Ground Floor Commercial Unit, Block E, 30 Indescon Square, Canary Wharf, London E14 9DR. You can contact us on 020 7531 6600 or at info@rejuvenceclinic.co.uk.
The Care Quality Commission (CQC) regulates Rejuvence Clinic as a registered provider.
Our aesthetic treatments are only for adults aged 18 and over. We may ask you to show photo ID, such as a passport or driving licence.
If you are bringing children, they need to be old enough to wait on their own. Children cannot accompany you into treatment rooms.
Before any treatment we ask you to fill in a health questionnaire. This is needed to plan your consultation and treatment safely. You must give us full and accurate information about your health, medicines, allergies, previous treatments, pregnancy or breastfeeding, and anything else your practitioner asks about, and tell us about any changes before each appointment.
If we do not have accurate information, treatment may not be safe. We may postpone or decline treatment if we think it is not suitable for you. If we decline treatment on clinical grounds, any deposit for that treatment will be refunded.
If you book and pay for a consultation or treatment without being at the clinic (for example online, by phone or by email), the Consumer Contracts (Information, Cancellation and Additional Charges) Regulations 2013 give you the right to cancel.
If you want your appointment to take place within the 14-day cancellation period, we must have your express request to start. We will ask you to confirm this when you book.
To cancel, tell us clearly that you want to cancel by phone on 020 7531 6600, by email to info@rejuvenceclinic.co.uk or by letter to the clinic address. You can use the model cancellation form at the end of these terms, but you do not have to. Online bookings cannot be cancelled through the booking system itself.
This legal right applies to bookings made at a distance. It does not apply to treatment agreed and booked in person at the clinic. For those bookings, our own cancellation policy in section 7 applies.
If you do not attend and have not told us in advance, we may keep the deposit for that appointment. We do not charge any further fee for a missed appointment.
If you arrive late, we may need to shorten your appointment or move it to another time, so that other patients are not kept waiting. We will always make sure there is enough time to treat you safely.
Some treatments work best as a course of sessions. Your practitioner will only recommend a course where it is clinically appropriate, and will tell you the full course price before you agree.
Every person responds differently. Outcomes depend on factors such as your skin, age, health, lifestyle, the number of sessions and how closely you follow aftercare advice. We cannot guarantee a particular result. At your consultation we will explain what results are realistic for you, and the risks and side effects.
We will provide our services with reasonable care and skill. If we do not, you are entitled to ask us to put it right, or, if that is not possible or not done within a reasonable time, to a price reduction, which may be a full refund. This is in addition to your other legal rights.
We will give you aftercare advice after your treatment. Please follow it, and contact us if you have any concerns. If you need a review appointment, we will arrange it with you.
If you have a problem after treatment, contact the clinic on 020 7531 6600. Outside opening hours, call NHS 111 for urgent medical advice, or 999 in an emergency.
Our privacy policy explains how we handle your personal and health information.
We will only send you marketing if you have agreed, or if you already have treatment with us and have not asked us to stop telling you about similar treatments. You can opt out whenever you like.
Nothing in these terms limits or excludes our liability for death or personal injury caused by our negligence, for fraud or fraudulent misrepresentation, or for anything else that cannot be limited or excluded by law.
We welcome feedback in person, by phone, by email or through any review platform. If you are unhappy with any part of your care, please tell us as soon as possible. Our complaints policy explains how to complain and what happens next. Making a complaint will not affect your care.
We may update these terms from time to time. The version that applies to you is the one in place when you made your booking. The date at the top shows when these terms were last changed.
These terms are governed by the law of England and Wales. You can bring legal proceedings in the courts of England and Wales. If you live in Scotland or Northern Ireland, you can also bring proceedings in your local courts.
Complete and return this form only if you wish to cancel a booking within your 14-day cancellation period (see section 6). You do not have to use this form. You can copy the text into an email or letter.
